Student Enrollment Requirements

The following documentation is required when enrolling your student(s)

PROOF OF RESIDENCY: 

• Parent/Guardian must bring a current utility bill; water, electricity or gas bill, rental agreement or mortgage statement 

BIRTHDATE VERIFICATION:

• Birth verification must be in the form of child's birth certificate, baptismal certificate, hospital certificate or passport to verify birthdate.

PHYSICAL EXAM:

• Children entering Transitional Kinder, Kinder, First Grade, or students coming from out of state, are no longer required to provide a physical exam for enrollment. It is highly recommended that one be done to make sure students are healthy and ready to start school. 
• A Physical Exam is required for children entering Preschool, and for students coming from outside the country. The CHDP form is to be signed and dated by a professional and must have a stamped seal from the clinic. 

ORAL HEALTH ASSESSMENT:

• Children entering Tansitional Kinder or Kindergarten should have a dental exam before enrolling. Oral health assessment form is to be signed and dated by a dental professional and must have a stamped seal from the clinic. 

VERIFICATION OF IMMUNIZATION:

Parent/Guardian must have immunization records with the following vaccines:

• *Polio - 4 doses
• *MMR - 2 doses
• *Varicella - 2 doses
• *Hepatits B - 3 doses
• *DTP - 5 doses

STUDENTS ENTERING 7th GRADE:

Parent/Guardian must have an immunization record card with all immunizations listed in TK and Kinder requirements PLUS:

• TDAP (whooping cough) - 1 dose  and   Varicella (chickenpox) - 2 doses 

Please provide the following (if available or applicable):

• Previous Elementary or Middle School transcripts if available
• Most current IEP, Psychological Report if student is receiving Special Ed Services
