Parents' Guide to Immunization Requirements for School Entry
Important Information for all students enrolling /entering TK / KINDERGARTEN AND 7th through 12th grade.
For the protection of all students attending schools within the Ontario Montclair School District, it is important that all immunization requirements under California School Immunization Law are met before a child begins school. These guidelines have been established by the California Department of Health Services and Centers for Disease Control and Prevention.
Students who are enrolled in OMSD Online Academy who are exiting the online program must be up to date with immunizations in order to start in-person instruction. *If your child is unimmunized due to medical reasons, please notify us.
