School Revenue Opportunity Form

  • At the beginning of every school year, all OMSD schools gather family information to help qualify for monetary funds. Vista Grande Elementary School needs ALL of our families to fill out the School Revenue Opportunity Form no later than September 30. This form looks a lot like the meal application form, but it is different and does not determine your child's eligibility for meals. This year ALL STUDENTS at Vista Grande will receive meals at NO charge. The QR code and link for the form are found below. If you need any assistance filling out the form, please call the office and we will be happy to help you.
