Out-of-district students:

Step 1: Submit an Inter-district transfer form to CWA Office (drop off or mail in to CWA Office, 950 W. D Street, Ontario CA 91762) or email to onlineacademy@omsd.net

• Forms are availalble from your district of residence and require a signature for release

Step 2:  Families will be notified if the transfer is approved

Step 3:  If approved, student will automatically be enrolled in Online Academy.

Step 4:  Parent & Student will attend required in-person materials pick-up & complete paperwork with classroom teacher and/or staff

• TK-8 Master Agreement
• Responsibility Agreement
• Registration Documents

A parent signature is required.

In-district students:
